Rates
$165 per hour session

Insurance
Services may be covered in full or in part by your health insurance or employee benefit plan. Before your first appointment, I will verify your benefits with your carrier and provide you with information regarding coverage prior to your first visit with me. You can also check your coverage carefully by asking your carrier the following questions:

1. •Do I have mental health insurance benefits?
   

2. •What is my deductible and has it been met?
   

3. •How many sessions per year does my health insurance cover?
   

4. •What is the coverage amount per therapy session?
   

5. •Is approval required from my primary care physician?
   

I take the following insurance plans:

1. Blue Cross Blue Shield
   

2. Humana
   

3. PHCS
   

Many insurance companies provide good out-of-network benefits, so if I don’t take your insurance, your visits may be still be covered by insurance.

Payment
Cash or check.

Cancellation Policy
If you fail to keep your scheduled therapy appointment, and you have not notified me at least 24 hours in advance, you will be required to pay the full cost (full copay if insured) of the session.
